On la trouve dans les tissus des mammif\u00e8res et elle est g\u00e9n\u00e9ralement extraite de la muqueuse intestinale ou d&rsquo;autres tissus appropri\u00e9s de mammif\u00e8res domestiques destin\u00e9s \u00e0 l&rsquo;alimentation humaine. Elle est purifi\u00e9e afin de conserver une combinaison d&rsquo;activit\u00e9s contre les diff\u00e9rentes \u00e9tapes de la coagulation sanguine. Elle est compos\u00e9e de polym\u00e8res de d\u00e9riv\u00e9s altern\u00e9s de D-glucosamine (N-sulfat\u00e9, O-sulfat\u00e9 ou N-ac\u00e9tyl\u00e9) et d&rsquo;acide uronique (acide L-iduronique ou acide D-glucuronique) li\u00e9s par des liaisons glycosidiques. Le fractionnement des h\u00e9parines permet d&rsquo;obtenir des h\u00e9parines de bas poids mol\u00e9culaire (HBPM) qui pr\u00e9sentent certains avantages par rapport aux h\u00e9parines non fractionn\u00e9es.    <\/span><\/p>\n<div class=\"blog-content\">\n<div class=\"field field-name-body field-type-text-with-summary field-label-hidden\">\n<div class=\"field-items\">\n<div class=\"field-item even\">\n<h3><\/h3>\n<h3><span style=\"font-size: 14pt;\">Histoire r\u00e9cente<\/span><\/h3>\n<p><span style=\"font-size: 14pt;\">En r\u00e9ponse \u00e0 la crise de falsification de l&rsquo;h\u00e9parine de 2007\/2008, qui a caus\u00e9 au moins 81 d\u00e9c\u00e8s aux \u00c9tats-Unis suite \u00e0 la contamination de la mati\u00e8re premi\u00e8re fabriqu\u00e9e en Chine par <b>du sulfate de chondro\u00eftine sursulfat\u00e9<\/b> (OSCS), les pharmacop\u00e9es am\u00e9ricaine et europ\u00e9enne ont r\u00e9vis\u00e9 leurs monographies sur l&rsquo;h\u00e9parine <b>.<\/b> Les monographies USP32-NF27 et Ph Eur. Ed. 7e <g id=\"gid_2\">\u00e9dition<\/g> ont int\u00e9gr\u00e9 l&rsquo;application de techniques analytiques orthogonales telles que <g id=\"gid_3\">la RMN du proton <g id=\"gid_4\">(\u00b9H<\/g> -RMN)<\/g> , <g id=\"gid_5\">la chromatographie d&rsquo;\u00e9change d&rsquo;anions<\/g> et <g id=\"gid_6\">l&rsquo;\u00e9lectrophor\u00e8se capillaire<\/g> afin de confirmer l&rsquo;absence d&rsquo;OSCS. De plus, le test de coagulation pr\u00e9c\u00e9demment r\u00e9alis\u00e9 sur plasma de mouton a \u00e9t\u00e9 remplac\u00e9 par des tests plus sp\u00e9cifiques d&rsquo; <strong>activit\u00e9 anti-facteur IIa<\/strong> et <strong>anti-facteur Xa<\/strong> .  <\/span><\/p>\n<h3><\/h3>\n<h3><span style=\"font-size: 14pt;\">Base scientifique du test<\/span><\/h3>\n<p><span style=\"font-size: 14pt;\">Le facteur Xa et le facteur IIa (ou thrombine) sont des prot\u00e9ines hydrolytiques (ou peptidases) qui interviennent dans la voie finale commune de la cascade de coagulation. Le facteur Xa catalyse la transformation de la prothrombine en facteur II. Le facteur II, quant \u00e0 lui, catalyse la conversion du fibrinog\u00e8ne soluble en fibrine insoluble, constituant ainsi le tissu fibrineux.  <\/span><\/p>\n<p class=\"rtecenter\"><span style=\"font-size: 14pt;\"><b>Prothrombine + Facteur Xa \u2192 Facteur IIa<\/b><\/span><\/p>\n<p class=\"rtecenter\"><span style=\"font-size: 14pt;\"><b>Fibrinog\u00e8ne + Facteur IIa \u2192 Fibrine (tissu)  <\/b><\/span><\/p>\n<p><span style=\"font-size: 14pt;\">L&rsquo;activit\u00e9 des facteurs IIa et Xa est inhib\u00e9e par l&rsquo;antithrombine (AT), modulant ainsi la cascade de coagulation&nbsp;: une augmentation de l&rsquo;activit\u00e9 de l&rsquo;AT entra\u00eene une diminution de la coagulation et inversement. L&rsquo;h\u00e9parine se lie \u00e0 l&rsquo;AT, induisant un changement conformationnel qui renforce son effet inhibiteur sur les facteurs IIa et Xa. Cette potentialisation de l&rsquo;inhibition de ces peptidases est \u00e0 l&rsquo;origine de l&rsquo;effet anticoagulant des h\u00e9parines lors de leur utilisation pharmacologique. Les h\u00e9parines de bas poids mol\u00e9culaire catalysent moins facilement la r\u00e9action entre le facteur IIa et l&rsquo;antithrombine que celle impliquant <b>le facteur Xa<\/b> .
